The Integrated Application Form (Notification Form) is a standard form used by foreigners residing in Korea when applying for or reporting various immigration-related matters, including foreigner registration, stay period extension, and change of status of stay. It is based on [Attached Form No. 34] of the Enforcement Rules of the Immigration Act (Ministry of Justice Decree No. 1106, partially amended and enforced on January 23, 2026). Because a single form can handle many different types of applications and notifications, it is called the “integrated” application form.

    Download the latest Integrated Application Form (Notification Form) from HiKorea (Korea Immigration Service's online portal) or the Ministry of Justice petition form page.

  2. 2. Check the Application Type

    Check only one box from the checkboxes at the very top of the form.

  3. 3. Fill Out the Form

    Fill in all relevant fields including personal information, passport information, and workplace information, and attach a photo.

  4. 4. Prepare Supporting Documents

    Prepare the required attachments according to the application type and status of stay.

  5. 5. Choose a Submission Method

    Apply online through HiKorea, or make a visit appointment at your local Korea Immigration Service office and submit in person.

  6. 6. Pay the Fee

    Pay the fee for your application type using revenue stamps (some applicants are exempt).

How to Fill Out the Form

The form may be filled out in Korean or English.

1. Personal Information

FieldInstructions
NameWrite exactly as it appears in your passport. Enter the Surname and Given names separately.
Date of BirthWrite in year (yyyy), month (mm), day (dd) order.
SexCheck Male (M) or Female (F).
NationalityWrite the country that issued your passport.
Alien Registration NumberFill this in only if you already have an Alien Registration Card. Leave it blank if you are registering for the first time.
Passport InformationWrite the passport number, issue date, and expiry date exactly as they appear in your passport.
Address in KoreaWrite your current address along with your phone number and mobile phone number.
Address in Home CountryWrite your home country address and contact information.

2. School Enrollment (for Students)

If your child is currently attending school, check the relevant box and write the school name and phone number.

  • Schools approved by the Office of Education: Regular elementary, middle, or high schools that have been officially licensed by the Ministry of Education or a regional Office of Education.
  • Unapproved alternative schools: Schools that are not officially licensed but still provide education (for example, alternative schools for migrant children).

  • Current workplace: Write the business name, business registration number, and phone number of your current employer.
  • Planned workplace: Write the information for the workplace you are moving to or adding.
  • Annual income: Write your total annual income in units of 10,000 won. (Example: 30,000,000 won — write 3000)

4. Attaching a Photo

Prepare a passport-size photo (35mm x 45mm) taken within the past six months and attach it in the designated box.

A photo is required only for foreigner registration and re-issuance of the Alien Registration Card. It is not needed for other types of applications.

5. Bank Account for Refund

Fill this in only when applying for foreigner registration or re-issuance of the Alien Registration Card. Write your Korean bank account number in your own name, in case a fee refund is needed.

This section allows the handling public official to look up necessary administrative information (such as business registration and resident registration records) directly. If you give your consent, you do not need to submit those documents separately. If you do not consent, you must prepare and submit the relevant documents yourself.

  • The applicant signs in person.
  • If necessary, a spouse or parent (in the case of a minor child) also signs together.

Documents to Submit Together

In addition to the Integrated Application Form, additional documents are required depending on the type of application and status of stay. Submission Methods

  1. Online Application: Sign up as a member at HiKorea (www.hikorea.go.kr) and apply electronically.
  2. In-Person Application: Submit directly at your local Korea Immigration Service office or branch office. Making an appointment on HiKorea before visiting will help reduce waiting time. From January 2026, visit appointments must be made directly by the applicant using their own HiKorea account.
  3. Mail Application: Available only for residents of island areas (remote islands); applications may be submitted by registered post through a post office.

Fees

Fees vary by application type and are paid by attaching revenue stamps to the back of the form. Fees are waived in certain cases.
